Job summary

Wellstar Health System is seeking a Care Coordinator RN to assess transitional care needs and coordinate care across the continuum. You will engage with patients and families to ensure needs are met, manage length of stay, and promote efficient utilization of resources.

The role involves psychosocial assessment, care progression, discharge planning, and collaboration with the care team to resolve barriers and expedite discharge. Strong teamwork and communication are essential.

Qualifications

• Required minimum education includes an Associate Degree in Nursing (ADN) or Diploma in Nursing; BSN preferred.
• RN license: RN - Single State or RN-COMPACT (multi-state).
• BLS certification required (Basic Life Support).
• Minimum 1 year nursing experience in the acute care setting.
• Excellent written and verbal communication; ability to work under stress in a fast-paced environment.

Responsibilities

• Assess patients' chronic disease management needs and psychosocial risk factors.
• Coordinate care progression and discharge planning with care team.
• Collaborate with physicians, PAS, financial counselor and UM nurse on insurance and coverage needs.
• Document and track discharge plans and ensure timely progression to discharge.
• Participate in interdisciplinary rounds and provide patient/family education.
