Job summary

Ochsner Health is seeking a registered nurse to support discharge planning and case management across the patient care continuum. You will review admissions, continued stay criteria, and collaborate with the care team to align with CMS and payer guidelines.

The role requires an RN license in the state of practice and BLS certification, with preferred CCM/ACM credentials and 3 years of hospital experience in discharge planning or utilization review.

Qualifications

• Graduate of an accredited school of nursing.
• Current registered nurse license in state of practice.
• Basic Life Support (BLS) certification required.
• Preferred certification in Case Management (CCM) or Accredited Case Manager (ACM).
• 3 years hospital-based experience in discharge planning, case management or utilization review preferred.

Responsibilities

• Performs pre-admission, admission, and continued stay reviews per CMS guidelines and payer policies.
• Leads discharge planning and coordinates care during acute-care hospitalization.
• Maintains open communication with ED physicians, hospitalists, and care team members.
• Acts as liaison between patients, caregivers, and post-acute/third-party payers.
• Ensures compliance with licensure, standards, and CMS reimbursement guidelines.
