• Develops a multidisciplinary discharge plan with the care team and post-acute providers.
• Oversees and evaluates the discharge plan implementation.
• Coordinates with care team and post-acute providers for timely transitions to appropriate settings.
• Delivers patient notifications and related documentation.
• Participates in performance improvement activities as needed.
