Dignity Health is seeking an RN Care Coordinator in Los Angeles to lead discharge planning and care coordination across the patient journey. You will work with physicians, nursing, insurers, and post-acute providers to ensure smooth transitions and optimal outcomes.

The role requires clinical nursing expertise, strong communication, and the ability to advocate for patients while navigating CMS guidelines and utilization management.

Formación

• Requires California RN license and BLS
• Experience with discharge planning and utilization review
• Strong knowledge of CMS standards and requirements
• Ability to work in fast-paced, team-driven environment

Responsabilidades

• Complete discharge planning assessment and update plan as needed
• Develop multidisciplinary discharge plans with care team and post-acute providers
• Oversee and evaluate discharge plan implementation
• Collaborate with care team and post-acute services for timely transitions
• Identify readmission risk and implement mitigation strategies for 30 days
