Job summary

You will assess, plan, and facilitate care across the continuum, advocate for patients, and collaborate with physicians, nursing, departments, insurers, and post-acute providers to ensure timely, high‑quality transitions.

Qualifications

• Minimum two years of acute hospital clinical experience or a master's degree in Case Management or Nursing in lieu of 1 year experience.
• California RN license.
• BLS certification from American Heart Association.
• BSN degree preferred.
• Five years of nursing experience preferred.
• CCM/ ACM-RN or UM Certification preferred.
• Knowledge of managed care and payer environment preferred.

Responsibilities

• Coordinate care across the continuum to optimize outcomes.
• Advocate for patients and ensure smooth transitions.
• Collaborate with physicians, nursing, departments, insurers, and post-acute providers.
