The role emphasizes multidisciplinary collaboration, education and advocacy, ensuring timely transitions to post-acute settings, reducing readmission risks, and promoting humankindness in care.

Qualifications

• Two years acute hospital clinical experience or a Master’s degree in Case Management or Nursing may substitute one year of experience.
• Graduate of an accredited school of nursing.
• Registered Nurse license: AZ or Compact.
• American Heart Association BLS within 90 days.
• Knowledge of managed care and payer environment.
• Strong critical thinking and problem-solving skills.
• Ability to collaborate with multiple stakeholders and teams.
• Understand utilization management and case management integration with CMS standards.

Responsabilités

• Completes and documents discharge planning assessments for patients identified by screening or on request.
• Develops multidisciplinary discharge plans with input from patients, families, and post-acute providers.
• Oversees and evaluates discharge plan implementation and progress.
• Collaborates with care teams to ensure appropriate inpatient resource use per evidence-based guidelines.
• Coordinates transitions to appropriate post-acute care settings based on clinical needs.
