Qualifications

• Completion of an accredited nursing program and an unrestricted RN license in any state; BSN preferred or ADN with BSN plan within four years of hire.
• Case management certification is preferred.
• Bilingual English and Navajo preferred.
• Must have a valid driver’s license and pass background check.

Responsibilities

• Perform chart reviews for upcoming appointments in patient panels.
• Track outstanding tests and results, and monitor consult outcomes.
• Conduct post-discharge follow-up calls for high-risk patients.
• Coordinate care between hospitals andWIHCC services.
• Maintain internal patient care database for continuity of care.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Coordinate multi-disciplinary care conferences for high-risk patients.
• Ensure compliance with HIPAA, confidentiality, and consent guidelines.
• Advocate for patients/families at service-delivery and policy levels.
• Develop and share best practices for panel management.
• Serve as clinic resource and coordinate with other service lines.
