Qualifications

• Completion of an accredited nursing program and an unrestricted RN license in any state.
• Bachelor’s degree or higher in Nursing; ADN with BSN within four years may be considered.
• Case management certification preferred.
• Bilingual English and Navajo preferred.
• Must maintain a valid driver’s license and pass background checks.

Responsibilities

• Maintains regular attendance and punctuality.
• Performs chart reviews for upcoming appointments.
• Tracks outstanding tests and results; monitors consult results.
• Conducts post-discharge follow-up calls for high-risk patients.
• Coordinates multi-disciplinary patient care conferences as needed.
• Maintains compliance with HIPAA, HITECH, and confidentiality standards.
• Acts as a resource nurse coordinating with other services.
