Job summary

MultiCare Health System is seeking a Nurse Navigator RN to provide comprehensive services across the care continuum. This hybrid role blends nursing, case management, and patient liaison duties to support clinical, quality and cost-effective outcomes for our patient population.

The Nurse Navigator will lead care coordination, engage in evidence-based practice, and collaborate with the care team to improve patient experience, safety, and costs.

Qualifications

• Bachelor's degree in Nursing; RN with 15+ years may be considered in lieu of degree.
• Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred.
• Current RN license in Washington State or Multistate License endorsement (MLS).
• Certifications such as Oncology Certified Nurse (OCN), Certified Case Manager (CCMC), Accredited Case Manager (ACMA), Care Coordination and Transition Management (AAACN), Ambulatory Care Nursing (AAACN), or equivalent.
• Three to five (3-5) years of recent clinical experience in area of specialty.
• Current BLS certification.

Responsibilities

• Organize patient care by prioritizing, delegating, delivering, and evaluating on an ongoing basis.
• Engage critical thinking in decisions regarding patient care with knowledge of policies, resources, and data.
• Utilize nursing education to deliver care using the nursing process per policies and protocols.
• Collaborate with the nursing team to improve quality of care, patient experience, and cost reduction.
• Foster a learning environment for patients/families, nursing staff, and other team members.
• Advocate for ethical and holistic care respecting diverse backgrounds.
• Track key quality metrics using national benchmarks where applicable.
• Lead care team initiatives to improve processes and outcomes aligned with the quadruple aim.
• Address non-clinical barriers by connecting patients with community services.
