Qualifications

• Bachelor’s degree in Nursing is required.
• Licensed to practice nursing in Arizona as RN.
• At least 3 years nursing experience.
• At least 2 years supervisory experience in healthcare or primary care.

Responsibilities

• Coordinate case management and care coordination activities.
• Monitor clinical quality measures and improvement initiatives.
• Assist in evaluating clinical staff skills and competencies.
• Provide oversight for immunization programs across multiple sites.
• Provide clinical triage support and contribute to patient care.
• Collaborate across departments to support integrated, patient-centered care delivery.
• Support the organization as needed.
