Job summary

Northern Valley Indian Health is seeking a Care Coordinator to manage a caseload for the Enhanced Care Management program, coordinating with care teams to improve patient outcomes.

You will build strong relationships with patients and families, conduct comprehensive assessments, and follow up to ensure services are effective and aligned with care plans. This role requires teamwork and strong communication in a community-focused setting.

Qualifications

• High School Diploma, GED or equivalent.
• 1+ year healthcare case management experience; 2 years preferred.
• Able to interpret complex care plans with sound judgement.
• Knowledge of clinical medical terms; behavioral health/SUD terms preferred.
• Strong collaboration and teamwork skills.
• Excellent written and verbal communication.
• Willingness to travel; valid CA driver's license.
• Advanced computer skills with EHR systems.

Responsibilities

• Manage caseload by conducting comprehensive assessments for ECM program.
• Develop and implement individualized care plans and follow-up.
• Coordinate with primary care, specialists, behavioral health, and community orgs.
• Educate patients and families about managing health conditions.
• Facilitate referrals, navigations, and linkages to community resources.
• Maintain documentation of assessments and outcomes per requirements.
• Build relationships with agencies and providers to advocate for patients.
• Assist team with escalations from patients and staff.
