Job summary

Under the direction of the ECM Program Director, you will conduct outreach, member engagement, clinical assessment, care planning, and coordination of services across providers to ensure timely, high-quality, patient-centered care. Travel within the service area is required.

Qualifications

• Minimum: ADN required.
• Preferred: BSN.
• Active, unrestricted California RN license.
• Driver’s license & auto insurance when using personal vehicle.

Responsibilities
