Job summary

Prominence Health seeks an RN Care Manager to provide management services for medically and socially complex members. Under supervision, you will develop individualized care plans and coordinate care with physicians, families, and community resources to optimize health outcomes.

The role emphasizes patient-centered, culturally sensitive care coordination and adherence to CMSA Standards of Practice for Case Management.

Qualifications

• Active, unrestricted nursing license in the States of Practice (Florida preferred).
• Minimum of three (3) years in clinical nursing practice and three (3) years in Case Management/Transition of Care.
• Strong communication skills and cultural sensitivity, with ability to work across diverse populations.

Responsibilities

• Develop and monitor comprehensive care plans for medically and socially complex members.
• Coach patients and families for adherence and self-care, including end-of-life planning.
• Collaborate with Prominence Health Plan contracted physicians and community resources to coordinate care.
