Job summary

CareSource/Commonwealth Care Alliance (CCA) in Massachusetts seeks an Integrated Care Clinical Manager to lead home-based, complex care management for dually-eligible enrollees with medical, behavioral and social needs.

The RN will manage assessments, care plans, and interdisciplinary teams to coordinate services across providers and community resources, promoting quality of life and reduced hospital utilization.

Qualifications

• AS/AA in nursing required; RN licensure required in MA.
• Prior experience in care coordination or case management with dual-eligible populations preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage with enrollees in their homes and community settings to establish complex care management relationships.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Coordinate with providers and community resources to ensure seamless communication and care transitions.
