Job summary

Prominence Health is seeking an RN Care Manager to coordinate care for medically and socially complex members. Under CMSA standards, you will develop individualized care plans, assess needs, and coach patients and families for adherence.

The role collaborates with physicians, promotes community resources, and supports SNP CM program implementation within UHS. Florida licensure and 3+ years in nursing/Care Management are required.

Qualifications

• Active, unrestricted RN license in the states of practice (Nevada, Texas and/or Florida).
• Minimum of three years of clinical nursing practice.
• Minimum of three years in Case Management/Transition of Care in managed care.
• Knowledge of CMSA Standards of Practice for Case Management.

Responsibilities

• Provide care management services for medically and/or socially complex members.
• Develop comprehensive care plans and coach for adherence to plans.
• Promote care management program to physicians and members; build community partnerships.
• Participate in SNP CM program implementation and ensure compliance with CMS/NCQA standards.
