Highland Hills Post Acute Perry SNF Healthcare LLC is seeking a skilled nurse case manager to collaboratively coordinate Plans of Care, PT/OT and other treatment protocols. Qualifications

• Bachelor’s degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.

Responsibilities

• Communicate with residents and families about plans of care and treatment protocols.
• Attend morning meetings to facilitate team communication.
• Monitor and adjust residents' statuses based on changing needs.
• Complete documentation promptly and accurately.
• Coordinate admissions, discharges and referrals.
• Perform utilization review to ensure appropriate, timely care.
• Work independently with strong judgment in interactions with physicians and payers.
