Responsibilities

• Communicate plans of care with residents and families and update as conditions change.
• Attend morning meetings to coordinate care with the team.
• Answer questions about care plans, illness progression and treatment.
• Organize daily work and prioritize resident needs.
• Complete documentation accurately and timely.
• Perform utilization review to ensure appropriate, cost‑effective care.
• Coordinate care with residents, providers, payers and facilities’ services.
• Oversee admissions and discharges and coordinate referrals.
