Job summary

Life Care Centers of America is seeking an RN Care Manager to coordinate patient care and transitions, collaborating with patients, families, care teams, payers, and external entities to ensure safe transitions in accordance with Life Care standards.

The role requires an active RN license and a nursing degree; one year of post-acute care experience is preferred, along with CCM or RN-BC certification being advantageous.

Qualifications

• Nursing diploma (associate’s or bachelor’s degree in nursing).
• Active RN license in good standing.
• 1 year of clinical post-acute care experience preferred.
• CCM or RN-BC certification preferred.

Responsibilities

• Coordinate patient care and transitions according to laws, regulations, and Life Care standards.
• Serve as liaison to external case managers, family, physicians, and community resources.
• Train and educate patients, families, and other providers of care.
• Implement and uphold standards of practice for care management and coordinated care.
