TITUSVILLE is seeking a qualified nurse to communicate plans of care, participate in team stand-ups, and ensure safe, cost-effective resident care. The role emphasizes documenting, coordinating with providers, and managing admissions and discharges in a high-volume clinical setting.

Ideal candidates have a nursing degree, an active RN license (LVN/LPN accepted), and at least two years of nursing experience, with knowledge of Medicare/Medicaid programs and PCC systems.

Qualifications

• Bachelor's Degree in Nursing or Social Work.
• Registered Nurse (RN) license preferred. Licensed LVN or LPN acceptable.
• Two (2) year clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.
• PCC knowledge.
• Must maintain continuing education/licensing.

Responsabilités

• Communicate regularly with residents and families about Plans of Care and treatment protocols.
• Attend morning stand-up meetings to facilitate team communication.
• Monitor residents' status and adjust care as needs change.
• Organize and prioritize daily work based on current and discharged residents' needs.
• Complete documentation and perform utilization review activities.
• Coordinate care with residents, care providers, facilities, and third-party payers.
• Oversee admissions and discharge activities.
