Job summary

PACS is seeking a dedicated nurse care coordinator to communicate plans of care, monitor resident needs, and coordinate with physicians, families, and payers in a high-volume facility in Louisiana. The role emphasizes timely documentation, utilization of Medicare/Medi-Cal knowledge, and seamless admissions/discharges.

The position requires RN or LVN/LPN licensure, with experience in clinical nursing and a CCM credential being a plus.

Qualifications

• Bachelor’s Degree in Nursing or Social Work.
• RN license preferred; LVN/LPN acceptable.
• Two year clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.
• Certificate as CCM a plus; continuing education/licensing required.

Responsibilities

• Communicate regularly with residents and families about Plans of Care and treatment protocols.
• Attend morning stand-ups to facilitate team communication.
• Answer questions about care, illness progression, and treatment plans to ensure safety.
• Monitor and adjust residents’ statuses based on changing needs.
• Organize daily work by assessing new, current, and discharge residents’ needs.
