PACS Group in the United States is seeking a skilled healthcare professional to coordinate care for residents in post-acute settings. You will communicate plans of care, answer questions, monitor changes, document accurately, and coordinate with families, providers, and payers to ensure safe, cost-effective services.

Strong clinical knowledge and independent judgment are essential. Ideal candidates hold an RN or LVN/LPN license (RN preferred) with related experience; CCM certification is a plus.

Qualificações

• Must hold a nursing or social work degree.
• RN preferred; LVN/LPN acceptable.
• CCM certification is a plus.
• 2+ years of clinical nursing experience preferred.
• Knowledge of Medicare/Medicaid programs and benefits.
• Ability to read and interpret policies and procedures.
• Strong communication and documentation skills.

Responsabilidades

• Communicate regularly with residents and families about care plans and treatment protocols.
• Attend morning meetings and coordinate with the team.
• Answer questions about care, progress, and plans to ensure safety and security.
• Monitor and adjust resident statuses based on changing needs.
• Organize daily work and prioritize tasks for high-volume environments.
• Complete documentation and perform utilization review activities.
• Coordinate care with residents, providers, payers, and facilities.
• Oversee admissions, discharges, and referrals.
• Maintain professionalism in interactions with residents, families, and staff.
