Job summary

PACS Group is seeking a qualified nursing professional for a full-time in Florida. The role involves communicating Plans of Care with residents and families, coordinating with the care team, and overseeing admissions and discharges to ensure timely, cost-effective care.

Qualifications

• Bachelor’s Degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN acceptable.
• Two years clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.
• Must maintain all required continuing education/licensing.
• Must have good written and verbal communication and documentation skills.

Responsibilities

• Communicate with residents and families about Plans of Care, treatment plans and progress.
• Attend morning meetings/stand-up to facilitate team communications.
• Answer residents’ questions about care and illness progression.
• Monitor and adjust residents’ statuses based on changing needs.
• Organize and prioritize daily work and assess new, current, and discharging residents’ needs.
• Complete required documentation in a timely manner.
• Perform utilization review activities for appropriate, timely care.
• Coordinate care with residents, providers, facilities’ financial services, and third-party payers.
• Oversee admissions and discharge activities and coordinate referrals.
• Maintain professional, respectful interactions with residents, families, staff and partners.
