Virginia Commonwealth University Health Systems is seeking an RN Care Coordinator to support a multidisciplinary team in transitioning patients from hospital to home or other care settings. The role ensures safe, efficient care across the continuum with emphasis on discharge planning and payer collaboration.

Required Virginia RN license or eligibility and 3+ years in acute care; master's degree preferred. This full-time role offers day/evening shifts in the Richmond area.

Qualifications

• Licensed Registered Nurse in the State of Virginia or eligible
• Minimum of three (3) years of nursing experience in an acute care setting
• Academic healthcare experience
• One (1) year of UM/Care Coordination experience
• Clinical experience with specialty patient population
• Baccalaureate Degree in Nursing or a Bachelor's degree in a biological/health or social sciences field
• Master's Degree in Nursing or a healthcare related field

Responsibilities

• Works with the attending and consulting physicians to facilitate effective and efficient transition through the hospitalization process.
• Collaborates with all members of the multidisciplinary team to coordinate care and ensure appropriate reimbursement by payers.
• Seeks the expertise of social workers to resolve psychosocial issues and develop complex transition/discharge plans.
• Acts as a liaison between the hospital and community agencies for information exchange.
• Maintains compliance with Quality Improvement Organization (QIO) and CMS guidelines.
• Uses medical necessity criteria to assess care level and setting, assists in denials and appeals.
• Participates in patient safety initiatives and upholds VCUHS Mission and Values.
• Establishes and maintains professional rapport with providers, patients, and internal customers.
