Qualifications

• Associate's Degree in Nursing or nursing diploma required at hire.
• California Registered Nurse License required at hire.
• Minimum 2 years of acute care experience in Medical-Surgical or Inpatient setting.
• At least 1 year of care management or utilization review experience, or completion of TIPS or Case Management Orientation upon hire.
• Bachelor's degree in nursing or higher within 3 years of hire (preferred).
• CCM or ACM certification (preferred).

Responsibilities

• Carry out effective case management functions for identified patients and families across the continuum of care.
• Communicate daily with medical staff, hospital personnel and outside resources to ensure appropriate resource utilization.
• Monitor medical records for variances to clinical and financial standards and intervene to achieve optimum outcomes.
