Qualifications

• Associate's Degree in Nursing or a nursing diploma upon hire.
• California Registered Nurse License upon hire.
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review or successful completion of TIPS program or Case Management Orientation Program upon hire.

Responsibilities

• Coordinate and manage care for complex patients across the continuum.
• Communicate daily with medical staff, hospital personnel, and outside resources.
• Monitor the medical record for variances to clinical and financial standards and intervene to achieve optimum outcomes.
