Job summary

Providence St. Joseph Hospital Eureka is seeking a Care Manager RN for a full-time role in Eureka, CA. The position is an 8-hour day shift and requires strong case management capabilities across the continuum of care.

Responsibilities include coordinating care, communicating with clinicians and outside resources, and monitoring records to ensure optimal clinical and financial outcomes. A CA RN license is required with 2 years of acute care experience.

Qualifications

• Nursing degree and CA RN license required.
• 2 years of acute care experience in Medical Surgical or Inpatient setting.
• 1 year in care management or utilization review, or completion of TIPS/Case Management Orientation.
• Bachelor's Degree within 3 years of hire; CCM/ACM certification preferred.

Responsibilities

• Provide case management planning, coordination, and navigation across continuum of care.
• Communicate daily with medical staff, hospital personnel, and outside resources to ensure appropriate resource utilization.
• Monitor medical records for variances and intervene to optimize patient and hospital outcomes.
