Job summary

Providence St. Joseph Hospital Eureka seeks an RN Care Manager to plan, coordinate and manage care for complex patients across the continuum. You will communicate with medical staff and outside resources to ensure optimal outcomes and alignment with clinical and financial standards.

The role requires CA RN license and 2 years of acute care experience; 1 year in care management is preferred. The position is full-time, day shift, on-site, with a comprehensive benefits package.

Qualifications

• Must hold a California RN license and a nursing degree/diploma.
• Minimum 2 years acute care experience in Medical/Surgical or inpatient setting.
• 1 year in care management or utilization review, or completion of TIPS/Case Management Orientation.

Responsibilities

• Carry out effective case management for identified patients across the continuum of care.
• Communicate daily with medical staff and external resources to ensure appropriate resource utilization.

Education

Associate's Degree in Nursing or nursing diploma

California Registered Nurse License

Bachelor's Degree in Nursing
