Job summary

Molina Healthcare is seeking an experienced RN Care Manager in Naperville, IL to support LTSS and care coordination. You will collaborate with a multidisciplinary team to ensure high-need members receive coordinated, cost-effective care and strive toward desired outcomes.

The role requires at least 2 years in health care with care management or LTSS, RN license in good standing, and ability to travel 25–40% locally. Strong communication and organizational skills are essential.

Qualifications

• 2+ years in health care, with 1+ year in care management or LTSS
• RN license active and unrestricted in practice state
• Valid driver’s license and reliable transportation

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans, including a waiver service plan in collaboration with members, caregivers, physicians and/or other health care professionals and member support network.
• Performs ongoing monitoring of care plan to evaluate effectiveness and document interventions and goal achievement.
• Promotes integration of services for members including behavioral health care and LTSS and home and community resources to enhance continuity of care.
• Assesses for medical necessity and authorizes all appropriate waiver services.
• Evaluates covered benefits and advises appropriately regarding funding sources.
• Facilitates interdisciplinary care team meetings for approval or denial of services and informal ICT collaboration.
