Job summary

Molina Healthcare is seeking a Care Manager RN to support LTSS care coordination in Belvidere, IL. This role collaborates with a multidisciplinary team to ensure member progress toward outcomes and effective care across the continuum, including waiver services.

The ideal candidate has at least 2 years in health care with care management and disability or chronic condition experience, active RN license, and ability to travel locally (25–40%).

Qualifications

• 2+ years of experience in health care including care management or managed care.
• Active RN license in state of practice.
• Bachelor's degree in health care-related field may be required in some states.
• Valid driver's license and reliable transportation for travel.
• Strong written and verbal communication skills.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans and waiver service plans with members and care team.
• Monitor care plans to evaluate effectiveness and adjust as needed.
• Promote integrated services across LTSS and behavioral health for continuity of care.
• Assess medical necessity and authorize waiver services as appropriate.
• Coordinate ICT meetings for approvals and collaboration.
• Travel 25–40% as required.
