Job summary

Molina Healthcare, a national health plan leader, seeks an RN to support care management and coordination for LTSS members across multiple Ohio counties. You will collaborate with a multidisciplinary team to develop and implement care plans, monitor progress, and promote integrated care including behavioral health services.

Travel up to 40% may be required. Required: active RN license, 2+ years health care and care management experience, and ability to work independently.

Qualifications

• 2+ years in health care, incl. 1 year in care management or LTSS
• RN with active license in state of practice
• Driver's license and reliable car for travel up to 40% of time
• Proactive, detail-oriented, strong communication and time management

Responsibilities

• Conduct comprehensive member assessments within timelines, including home visits
• Facilitate waiver enrollment and disenrollment processes
• Develop and monitor care plans with members, caregivers, and providers
• Coordinate services across LTSS and behavioral health to ensure continuity of care
• Assess medical necessity and authorize waiver services
• Lead ICT meetings for service approvals/denials
• Identify barriers and coordinate psycho/social/financial support
