Molina Healthcare, Inc. is seeking a Care Manager RN to support LTSS care coordination across multiple counties in Ohio. The role requires coordinating with a multidisciplinary team to deliver high-need member care and achieve desired outcomes.

You will assess, plan, and implement care with members, caregivers and physicians, monitor progress, and adjust plans while promoting integration of services including behavioral health and community resources. Travel 25–40% is expected.

Qualifications

• Active RN license in state of practice.
• 2+ years in health care with care management experience.
• Experience with LTSS and disabilities or chronic conditions.
• Willingness to travel 25–40% as required.

Responsibilities

• Completes comprehensive member assessments within timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and care teams.
• Monitors care plan effectiveness and adjusts as needed.
• Coordinates LTSS with behavioral health and community resources.
• Assesses medical necessity and authorizes waiver services.
• Participates in ICT meetings for service approvals.
• Provides guidance and support to peers as needed.
