Job summary

SAC Health in San Bernardino, CA is seeking an ECM Care Manager, RN to coordinate care for high-risk patients, including the homeless, chronic conditions, behavioral health, and social determinants. You will develop plans of care with the provider and engage patients across medical, behavioral, and social needs.

The ideal candidate holds an ASN in Nursing (BSN preferred), CA RN license, CPR/BLS, EPIC certification, and 3+ years of acute care experience.

Qualifications

• Associate of Science in Nursing required; BSN preferred.
• California RN license, CPR/BLS certification, EPIC training.
• 3+ years acute care hospital-based experience; bilingual preferred.
• Proficient with MS Office and electronic health records.

Responsibilities

• Review EMR to develop and evaluate individualized care plans with providers.
• Coordinate with care managers, program managers, and health professionals.
• Collaborate with physicians, therapists, social services and families on care plans.
• Support patient activation toward health goals using evidence-based practice.
• Monitor utilization and track medical/behavioral health outcomes.
• Lead and mentor care management team members; participate in meetings.
