Job summary

Health and Life Organization, Inc. dba Sacramento Community Clinic seeks an experienced RN case manager to coordinate comprehensive ECM services, lead team collaborations, and develop care plans for high-need patients.

You will conduct home visits, assess health needs, and ensure documentation supports program goals. The role requires a state RN license, one year of case management experience, and strong communication with diverse populations.

Qualifications

• RN license required and active in the state.
• Minimum 1 year of case management experience.
• Experience working with underserved patient populations preferred.
• Valid driver’s license with clean record.
• Successful completion of fingerprinting/livescan background check.

Responsibilities

• Oversee ECM benefit program care delivery and staff integration.
• Develop partnerships and care models with community-based health practices.
• Lead interdepartmental collaboration and communication for ECM services.
• Conduct staff meetings and participate in staff selection and training planning.
• Ensure ECM components align with contractual requirements and documented standards.
• Document patient interactions, health assessments, and home-visit findings for care teams.
