Job summary

SAC Health seeks an ECM Care Manager, RN in Indio, CA to coordinate clinical care for high‑risk patients. You will develop and monitor care plans with providers and multidisciplinary teams, addressing medical, behavioral, and social needs.

Ideal candidates have 3+ years acute care experience, CA RN license, and EPIC certification; bilingual in English/Spanish is preferred. Excellent teamwork, leadership, and patient advocacy are essential for this role.

Qualifications

• Education: ASN Nursing required; BSN preferred.
• Licensure: CA RN license, CPR/BLS; EPIC cert required for hired module.
• Experience: 3+ years acute care hospital experience.
• Skills: Proficient in MS Office; ability to use EHRs and databases.
• Interpersonal: Strong communication, teamwork and leadership.
• Work eligibility: Authorized to work in the United States; no sponsorship needed.

Responsibilities

• Review electronic medical records to plan and implement patient care with providers.
• Coordinate with case managers and health professionals for comprehensive care.
• Collaborate with physicians, therapists, social services, and families.
• Support patient activation toward health goals; provide evidence-based care.
• Triage and prioritize patient cases; oversee care team performance.
• Monitor utilization of resources; track health outcomes.
