Job summary

SAC Health in Indio, CA seeks an ECM Care Manager, RN to coordinate care for high-risk patients across medical, behavioral, and social domains. You will develop plans of care with providers and guide a multidisciplinary team.

The role entails mentoring Care Management Team members, coordinating with case managers and physicians, and ensuring efficient utilization of resources while documenting outcomes.

Qualifications

• RN with California license is required.
• 3+ years of acute care or hospital-based experience.
• Bilingual in English and Spanish preferred.

Responsibilities

• Review patient records to develop individualized care plans with providers.
• Coordinate with IEHP and care teams to implement recommendations.
• Collaborate with physicians, behavioral health, social services to plan care.
• Promote patient activation and evidence-based care.
• Mentor and train care management team members.
• Monitor utilization of resources and track outcomes.
• Oversee Patient Navigator and Coordinators to ensure cohesive teamwork.
• Ensure documentation meets medical necessity criteria.
