Job summary

SAC Health is seeking an ECM Care Manager, RN to clinically coordinate high-risk patients across medical, behavioral, and social needs. The role involves care planning, collaboration with a multidisciplinary team, and mentoring junior staff.

Schedule is 5 days, 8 hours, on-site in San Bernardino, CA. The RN will oversee care coordination, utilize EMR systems, and ensure evidence-based practice while improving patient outcomes.

Qualifications

• Associate of Science in Nursing required.
• California RN license required.
• CPR/BLS certification required (AHA or Red Cross).
• EPIC certification for assigned module required.
• 3+ years of acute care hospital experience.
• Bilingual in English and Spanish preferred.
• Must be legally authorized to work in the United States full-time.

Responsibilities

• Review patient medical records to coordinate quality care and plan of care with providers.
• Communicate with other care managers and health professionals to coordinate patient care.
• Collaborate with physicians, therapists, social services, and families to develop a coordinated plan.
• Support patient activation toward health goals and evidence-based practices.
• Assist in developing clinic goals and continuum of patient care.
• Monitor resource utilization and clinical outcomes across care settings.
• Triage and prioritize patient cases for population/caseload management.
• Oversee Care Coordinators and Patient Navigators on the team.
