Job summary

CareSource in Massachusetts is seeking an Integrated Care Clinical Manager, an RN responsible for monitoring, follow-up and care management for dually eligible enrollees with complex medical, behavioral, and social needs. The role focuses on coordinating health services and community resources to improve outcomes and quality of life.

Qualifications

• AS degree in nursing required; RN with ability to independently serve people with complex needs.
• Prior experience in care coordination, case management, or dual-eligible populations preferred.
• Medicaid and/or Medicare managed care experience preferred.

Responsibilities

• Engage with enrollees in home and community settings to establish a complex care management relationship.
• Function as liaison between healthcare providers, community resources, and enrollees for seamless communication and care transitions.
• Perform comprehensive assessments and engage enrollees in care plan development and updates.
