Job summary

CareSource in Massachusetts is seeking an Integrated Care Clinical Manager (RN) to lead complex care management for dually-eligible enrollees, coordinating medical, behavioral and social services.

This mobile position requires travel to member homes and community settings, collaborating with an interdisciplinary team, and ensuring adherence to NCQA standards to improve health outcomes and quality of life.

Qualifications

• RN license in good standing and ability to independently serve people with complex medical, behavioral and social needs.
• Associates degree in nursing (AS) from an accredited program.
• Case management experience with dual-eligible populations is preferred.

Responsibilities

• Engage enrollees in care at home and in community settings, addressing medical and social needs.
• Lead interdisciplinary care team to develop holistic care plans.
• Coordinate care transitions among providers and community resources.
• Perform comprehensive assessments and ensure timely follow-ups.
• Educate members about benefits and management of chronic conditions.
• Promote self-management and preventive care; screen for social determinants of health.
