Job summary

CareSource/Commonwealth Care Alliance (CCA) in Massachusetts is seeking an Integrated Care Clinical Manager. This role is a community-based RN position focused on monitoring, follow-up and clinical care management for dually-eligible enrollees with complex medical, behavioral, and social needs.

You will lead the interdisciplinary care team, coordinate with healthcare providers and community resources, and educate members about benefits, medication adherence and preventive care to improve

Qualifications

• AS degree in nursing from an accredited program is required.
• Current unrestricted RN license in the Commonwealth of Massachusetts is required.
• Experience in care coordination, case management, or working with dual-eligible populations is preferred.

Responsibilities

• Engage enrollees in homes and community settings to build a complex care management relationship.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Coordinate with providers, community resources, and state agencies to ensure seamless care transitions.
