Job summary

AdventHealth in Riverview, FL is seeking an experienced Registered Nurse in Case Management to coordinate discharge planning and ensure timely transitions of care across the multidisciplinary team. You will assess patient and family needs, educate on illness impact and available resources, organize care conferences, document plans and communicate with payors to obtain post-acute authorizations.

Qualifications

• RN licensure required.
• BLS/ CCM/ ACM credentials preferred.
• 2+ medical/hospital nursing experience required.
• Care management/utilization management experience preferred.

Responsibilities

• Actively participates in multidisciplinary rounds to review patient status, discharge plans and resource needs.
• Educates patients and families on illness impacts and available resources; advocates for timely discharge.
• Organizes and facilitates discharge planning conferences with the team.
• Documents discharge plans and barriers; communicates with payors for post-acute authorization as needed.
• Assesses readmission reasons and develops contingency plans.
• Reviews medical records to integrate clinical, social, and financial factors into care transition.
