Description

Care Manager RN at St. Joseph Hospital in Eureka, CA. This position is Per Diem and will work 8‑hour, Variable shifts.

Providence St. Joseph Hospital, Eureka, is a facility recognized for its excellence in patient care. The RN Care Manager is responsible for carrying out effective case‑management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. The role involves communicating on a daily basis with medical staff, hospital personnel and outside resources to ensure appropriate utilization of resources in accordance with clinical and financial standards. The RN monitors the medical record for variances to standards and intervenes to achieve optimum outcomes for the patient and the hospital.

Required Qualifications

• Associate’s Degree in Nursing or nursing diploma, and California Registered Nurse License upon hire.
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review in any setting, or successful completion of a TIPS program or Case Management Orientation Program upon hire.

Preferred Qualifications

• Bachelor’s Degree in Nursing or higher within 3 years of hire.
• CCM or ACM certification.

Benefits and Compensation

Pay Range: $54.61 – $92.23 per hour. Additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
