Job summary

Qualifications

• California RN license required upon hire.
• 2 years of acute care experience in Medical Surgical or Inpatient setting.
• 1 year of care management or utilization review experience, or completion of a Case Management Orientation Program.

Responsibilities

• Carry out effective case management for identified patients across the continuum of care.
• Coordinate, plan, negotiate, procure and manage care and needs of complex patients and families.
• Communicate daily with medical staff, hospital personnel and outside resources to ensure appropriate resource utilization.
