Job summary

CareSource seeks a Clinical Care Manager to provide in-home and community-based care management for dually-eligible enrollees with complex medical, behavioral, and social needs. You will coordinate with healthcare providers and community resources to improve health outcomes and quality of life.

Qualifications

• Associate degree in nursing from an accredited program required
• Master’s degree in social work or mental health counseling with independent license required
• Three years as RN/BH clinician or one year as RN/BH with two years of experience in dual-eligible populations as LPN/CHW/MA preferred
• Prior experience in care coordination or case management with dual-eligible populations preferred
• Medicaid and/or Medicare managed care experience preferred

Responsibilities

• Engage enrollees in homes and community settings to establish complex care management relationships
• Coordinate care between healthcare providers, community resources and enrollees
• Lead interdisciplinary care team to create holistic care plans
• Oversee enrollee utilization of long-term services and supports to keep them in their chosen location
• Educate members about benefits and managing chronic conditions and preventive care
