Job summary

McLaren Health Care seeks a Care Coordination RN to ensure proactive patient transitions to appropriate levels of care across the continuum. You will partner with physicians, nursing and care teams to improve discharge planning while targeting LOS and readmission reductions.

The role emphasizes collaboration with social workers and utilization management to optimize transitions and resource use. A valid RN license and a bachelor’s degree in nursing are required.

Qualifications

• State licensure as a Registered Nurse (RN).
• Bachelor’s degree in nursing from an accredited institution; or actively pursuing and to be obtained within five years of accepting position.
• Three years of acute hospital care experience.
• American Case Management Certification (ACM) or obtain certification when eligible as defined by the Association Case Management Association; maintenance of continuing education requirements.

Responsibilities

• Performs care coordination assessments within 24 hours of admission; conducts readmission and transition planning.
• Collaborates with social workers and disciplines to ensure safe, timely transition to next level of care.
• Assesses patient/family needs to reduce barriers and formulate discharge plans.
• Identifies unsigned level of care orders; communicates with utilization management nurse and providers for orders.
• Reviews DRG/LOS to assess discharge planning needs and identifies the primary contact.
