Job summary

This role requires at least 2 years in health care, active RN license, clean driving record, EMR familiarity, and strong communication. Travel up to 40% may be required to meet state contracts.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent.
• Active RN license in state of practice.
• Valid driver's license and reliable transportation for travel requirements.
• Understanding of EMR and HIPAA.
• Knowledge of community resources.
• Ability to work independently and manage multiple projects.
• Proactive, detail-oriented, and strong communication.
• Proficiency in Microsoft Office and navigating portals.

Responsibilities

• Completes comprehensive assessments of members per timelines to determine care management eligibility.
• Develops and implements care coordination plans with member and health team.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plans, documents interventions, and tracks goal achievement.
• Maintains ongoing member caseload and regular outreach.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and provides coordination to address concerns.
• RN may handle complex cases and medication reconciliation as needed.
• Travel 25-40% may be required.
