Job summary

East Bay Community Action Program in Barrington is seeking a Nurse Care Manager (RN) to coordinate care across medical and behavioral health services, bridging gaps and advocating for patients within our Behavioral Health program.

The role combines direct nursing, care planning, patient education, and collaboration with a multidisciplinary team to improve health outcomes and access to services for individuals with complex needs in our community.

Qualifications

• RN license issued by Rhode Island Department of Health.
• Associate's Degree in Nursing or equivalent.
• Experience providing nursing care in clinical or community settings.
• Experience coordinating care for patients with complex medical/behavioral needs.

Responsibilities

• Perform comprehensive patient assessments and identify barriers to care.
• Create individualized care plans addressing medical and behavioral health needs.
• Coordinate services with providers, hospitals, pharmacies and community agencies.
• Monitor progress and adjust interventions for better outcomes.
• Educate patients on disease management and self-management strategies.
• Maintain accurate documentation in the EHR.
• Collaborate with multidisciplinary teams and participate in quality improvement.
