Job summary

MedCadre is seeking a Registered Nurse Care Manager to coordinate patient care across healthcare services. You will assess patient needs, develop care plans, and coordinate services to support quality outcomes.

In this role you will collaborate with providers, educate patients and families on treatment plans, facilitate referrals and transitions of care, and maintain accurate documentation to ensure safe, coordinated care.

Qualifications

• Active RN license.
• Graduate of an accredited nursing program.
• Previous care management, case management, or clinical nursing experience preferred.
• Strong assessment, care coordination, and critical-thinking skills.
• Excellent communication and organizational skills.
• Ability to manage multiple patients and coordinate complex care needs.
