Responsibilities

• Perform treatment planning, update assessments, and follow up to connect clients with healthcare and community resources.
• Navigate care systems by connecting clients with the right teams and resources.
• Maintain encounter notes detailing service purpose, actions taken, and outcomes.
• Address client crises promptly and facilitate resolutions or escalations.
• Collaborate with multidisciplinary teams and coordinate housing, employment, education, and healthcare services.
• Contribute to discharge planning and transitions to appropriate care levels.
• Engage clients in group and individual sessions.
