Job summary

Allegheny Health Network is seeking a Care Management Education Leader to design and evaluate onboarding, annual education, continuing education, and competency validation across care settings. Travel to multiple hospital sites supports education, onboarding, and competency activities.

Qualifications

Master's degree in Social Work, Nursing, Education, or equivalent.
2 years hospital-based Care Management experience.
2 years Education and training experience.
Current PA RN or PA SW licensure or equivalent via eNLC within 3 years.

Responsibilities

Collaborates with leadership to support orientation, ongoing education, quality improvement initiatives, and practice excellence in transitions of care and care coordination.
Plans, develops, implements, and evaluates Care Management education programs with measurements to assess learning and competency outcomes.
Serves as an educational consultant to leadership on regulatory requirements and program development.
Coordinates onboarding programs including role-specific orientation and continuing education initiatives.
Conducts learning needs assessments and manages annual competency and education documentation with leadership.
