The ideal candidate will have a Bachelor’s Degree in Nursing and at least three years experience in a clinical nursing role, along with current RN licensure in the state of PA. Compliance with HIPAA and continued education is essential.

Qualifications

• 3 years in a clinical nursing role required.
• Current State of PA RN licensure or multi-state licensure required.
• CPR certification from American Heart Association needed.
• Must pass Act 34, 33, and 73 clearances.

Responsibilities

• Assess patient needs for effective care transition planning.
• Facilitate development of continuum of care transition plans.
• Document and validate data to monitor interventions and outcomes.
